As a member of Community Health Plan, CHC accepts Basic Health Plan and Healthy Options. We also accept Medicaid, Medicare and may accept other private insurances. Please check with your clinic for details.
Medicaid and private dental insurance are accepted. Medicare and Basic Health do not pay for dental services. Note: Please contact your insurance carrier directly if you have any questions on your coverage. You are responsible for any non-covered services.
We will help you determine your eligibility for various programs offered by the State. If you do not qualify, we offer discounted fees based upon family size and income level. Please bring completed application and proof of income (for the last 30 days) with you at the time of your visit.
Community Health Center of Snohomish County does not refuse service to anyone based on ability to pay.
