Making An Appointment

To see a Behavioral Health Specialist, you will first need to make an appointment with your Primary Care Provider (PCP). Your PCP will assess your needs and recommend if you should make an appointment with the Behavioral Health Specialist.

Our Behavioral Health Specialists are trained in counseling and other behavioral therapies. They work closely with your PCP, and can provide counseling at the same time you are receiving medication for mental health. Behavioral health service is available to adults who are medical patients at CHC and who cannot access other mental health support in the community.

  • Medical insurance card
  • Medicaid coupon
  • Medicare card
  • Proof of income (pay stubs, tax return)
  • Photo ID
  • A list of all medications you are taking (or bring the medications with you)
