Change in our Electronic Health Record System

CHC has made a change to its Electronic Health Record system. This means a few things will look different and we ask for your patience and understanding as we learn the new system to better serve you.

– Patient Portal:

The look and feel to our online Patient Portal will be different.

– Medical Statements:

Medical statements will also look different. You will still have the option to pay

either by mail or make an online payment.

– Medical service payments:

Medical service payments will need to be sent to a different address. Please look for the address on your statement. **Dental service payments will remain the same**

We ask that you bring your insurance card and picture id with you to your next appointment so that we may serve you better in an efficient way.
