Medical Plans

Washington Apple Health and Medicaid

Apple Health is Washington State’s Medicaid program that offers free or low-cost insurance coverage for those who qualify. Apple Health covers primary care, emergency visits, maternity services, pediatric care, dental services, vision care, prescription medications, and more.

Enrollment in Apple Health is open year-round, allowing applicants to apply for coverage at any time. However, Apple Health members are required to renew their coverage annually. Fill out an application on Washington Health Plan Finder to see if you qualify for one of the following Apple Health plans.

No insurance? We can help!

CHC does not refuse service to anyone based on ability to pay or for any reason. We can help you determine your eligibility for various state and federal programs and sign you up for our Sliding Fee Discount Program. A Patient Services Specialist is available at each of our locations to assist you with filling out the required forms.

Sliding Fee

Whether or not you have insurance, we may be able to reduce your costs through our Sliding Fee Discount Program. Eligibility is based on income and household size. We offer discounts off our regular charges based on a person’s income and the number of people in their household. All patients are encouraged to complete an application. Please remember to bring proof of income.

Sliding Fee Discount Eligibility Guidelines

Follow these steps to determine patient responsibility or the level of discount available:

  1. Find the size of your family in the left-hand column
  2. On the row of family size, find your household’s annual income
  3. Once you’ve located your income level, go to the top of the column to find the patient responsibility or level of discount.
  4. Full payment of your portion is expected at the time of service.
