What To Bring On Your First Visit

  • Medical insurance card, Medicaid coupon and/or Medicare card
  • Proof of income (pay stubs, tax return)
  • Photo ID
  • A list of all medications
Our Behavioral Health Specialists are trained in counseling and other behavioral therapies. They work closely with your PCP and can provide counseling at the same time you are receiving medication for mental health. Behavioral health service is available to adults who are medical patients at CHC.
